Dear New Patient,

Welcome to our office for your oral and maxillofacial surgery needs. Our goal is to make your visit here as comfortable as possible.

Complete the Secure Online Patient Registration forms as soon as possible. Completing your paperwork at home will help our team verify any insurance eligibility and help complete your registration before you arrive. If you cannot complete these online forms, please come to our office 45 minutes prior to your initial scheduled time to fill out your paperwork or call us to request that we mail them to you. We can also scan all dental and medical insurance cards and a photo ID upon your arrival. As a courtesy to you we will be happy to submit insurance claims; however, you must have your complete insurance information on the forms.

Please arrive 20 minutes prior to your scheduled time to check-in and have any necessary x-rays taken. If you arrive later than your appointment time you may be asked to wait longer than usual or it may be necessary to reschedule your appointment. If for any reason you cannot keep this appointment, please call our office at least 48 hours in advance. There may be a charge for a missed appointment.

A payment will be necessary at the time of your appointment even if insurance coverage is expected. Due to the increase in surcharges, all credit card payments will be charged a 2% service fee.

Patients of minor age (17 years old and under) must be accompanied by a parent or legal guardian. There are NO Exceptions to this policy.

For SURGERY Appointments ONLY

If you desire I.V. sedation for your surgical procedure, do NOT EAT OR DRINK for EIGHT (8) HOURS BEFORE SURGERY. Daily medications prescribed by your doctor should be taken with a sip of water unless directed not to do so. You must bring a responsible adult or a legal guardian if you have one or are a minor. They must wait for you in our office, drive you home and monitor you for the next 12 hours. Most insurance companies have special criteria for I.V. sedation to be a covered benefit. Please remove contact lenses before your surgical visit.
